I authorize myself/my child to participate in a karate class at White Wolf Martial Arts. I understand injuries can and do occur and hereby release White Wolf Martial Arts, it's staff, employees and members from liability for any injuries or damages occurring while at White Wolf Martial Arts facility or otherwise, except where such injury is caused by the gross negligence while directly providing service to the attendee. In the event of an emergency I authorize any licensed medical personnel to perform any accepted medical procedure deemed necessary at my expense. I consents to the use of my/my child’s image in photographs, video or other recordings taken during participation in or instruction at White Wolf Martial Arts for use in advertising, promotion, and marketing by White Wolf Martial Arts.
