I acknowledge and accept that martial art is a dangerous sport and involves the risk of serious injury. I hereby warrant that I am physically & medically sound to exercise, and have voluntarily elected to do so.

I acknowledge that whilst at White Wolf Martial Arts, both my property and my person shall be at my own risk, and I will not hold White Wolf Martial Arts or its staff, directors or volunteers liable for any personal injury or loss of property arising whilst on these premises.

Any of the above Terms and Conditions may be changed at the discretion of White Wolf Martial Arts and management at any time. Notification of such changes will either be via email out or posters displayed on our notice board.

I have read and understood the above and agree to all terms and conditions of the agreement.
